Notes to the PRAGUE CARD Insurance Policy

By filling in personal data into the respective part of the Prague Card & Prague Passport a contractual relationship is established between the Holder of this Prague Card & Prague Passport (hereinafter referred to as the Policyholder) and Generali Pojišťovna a.s. (Generali Insurance Company) (hereinafter referred to as the Insurer). 

The Policyholder acknowledges by the Insurance Policy activation (see Point 2 ) and by filling in his/her personal data into the respective sections (see Prague Passport, Page 3) that the Policyholder understands and agrees with the General Travel Insurance Conditions effective from 1 September 2003 and with the Special Travel Insurance Conditions as a part of the contractual relationship. The above Conditions are detailed in the offer of the Prague Card Service, and they are available with the Prague Card & Prague Passport distributors and on the Internet at www.praguecard.info.

 

   
By purchasing the Prague Card & Prague Passport the Policyholder agrees that his/her personal data and health information can be processed by the administrator of Generali Pojišťovna a.s. and by its contractual partners in accordance with the provisions of Act No 101/2000 Coll. for the purpose of insurance and other activities specified by Act No. 363/1999 Coll. on Insurance Industry for the period necessary to ensure rights and obligations resulting from the contractual obligations and for the period resulting from the governing legislation.

The Policyholder declares that he/she was informed about his/her rights and about obligations of the data administrator (in particular about the right to access personal data) as referred to in Article 11 of Act No 101/2000 Coll. and about other rights as per Article 21 of this Act.

NOTE: The Insurance is valid only for a tourist stay and covers only tourist activities. No indemnification can be provided by the Insurance Company to cover deliberate crimes, and any acts of violence and public nuisance, which could have been avoided by the Policyholder!

Insurance validity and activation

1. The Insurance covers the whole territory of the Czech Republic and is valid for the maximum period of 7 days (i.e. on the day of activation + 6 consecutive calendar days)!

2. Insurance activation

a) By having a Czech border control stamp with a date in the Prague Passport on page 3 when entering the Czech Republic,
b) or - by entering the date into and signing the Prague Card & Prague Passport,
c) or - by stamping the “Prague Card Ticket” for the Prague Public Transport in a orange ticket stamping machine to be found in all public transport.

3. The Insurance is not valid unless one of the above activation methods is applied (Point 2).
